Vriska still had her arm and eye while she was her dreamself, and I'm pretty sure Tavros still had working legs, could it be concluded that trans (ftm and mtf) characters would have their preferred bodies, so to say?

I don’t see why not. If Jade can grow extra arms it should be within the realm of possibility for a trans youth to alter their form as they see fit.
Does one have to be at the top of the echeladder before ascending to the god tiers or do you just have to die on your quest bed/sacrificial slab regardless of which ring you're at.

You just have to die at your thing. Most people are already at the top of their ladder by then but I don’t think it’s a requirement

The troll would only be in one of those quadrants at a time. Vacillating means to alternate.
