Well, in an alternate timeline it did, apparently. I believe in Roxy: Wake Up we see Godtier Karkat and Dreamer Nepeta together in a dream bubble. So our ship did sail, if even for a few brief moments.

I do not, that has not been explained in canon.  It could have something to do with his Jokerkind Strife Specibus or some Time Shenanigans, but again, I’m not sure.

She was killed by Sollux under Vriska’s mind control. And she came back as her Dreamself when it ascended.
